LA R1-99-29 | 4/26/1999 | Recommended that the Call Quality Monitoring Form and Account Information Sheet agreed to in LA 98-03 be replaced with the attached forms in this LA. The forms eliminate a number of redundant factors, combine some factors and clarify other factors.
